The hotel is very clean and comfortable, but I recommend earplugs, as rooms with windows facing the street can be loud at night (ask for a room with windows facing east). I have stayed here twice, and had a small issue checking in this time, but the staff are very nice. The people living rough in the Tenderloin can be loud and dirty, but I've never felt threatened or scared, but I wouldn't stay here with small children who've never dealt with this reality.
Quiet, clean, expansive room with a very comfortable bed. My dog was welcomed without fee, a major asset for my travel plans. The neighborhood is "terrible" only because so many of our fellow humans live on these neighborhood streets. If you need an inexpensive but nice room in the area, this is a great option, I've already booked another night for a future visit.
